The role requires a PharmD, active pharmacist license (or pending within 120 days), ACLS/PALS if applicable, and at least 1 year of recent experience in diverse pharmacy settings.

Qualifications

• PharmD from an accredited pharmacy school.
• Registered Pharmacist within 120 days of hire.
• 1 year of recent relevant experience.
• Knowledge of medical terminology, pharmacology, and laws and regulations.
• Knowledge of NABP, TJC, Title XXII, DHS, DEA, FDA, and USP.

Responsibilities

• Deliver pharmaceutical care through patient-centered clinical services.
• Provide medication information and education.
• Prepare and distribute medications safely, effectively, and cost-efficiently.
• Provide input to and implement patients’ plans of care based on diagnosis.
• Assess patients and monitor care.
• Communicate with patients, families/support groups, and healthcare providers.
• Adhere to local, state, and federal regulations, codes, policies, and procedures.
• Travel between Sacramento Valley Float Pool sites as required.
