Job Responsibilities

• Deliver pharmaceutical care through patient-centered clinical services
• Provide medication information and education
• Prepare and distribute medications safely, effectively, and cost-efficiently
• Provide input to and implement patients’ plans of care based on diagnosis
• Assess patients and monitor care
• Communicate with patients, families/support groups, and healthcare providers
• Adhere to local, state, and federal regulations, codes, policies, and procedures
• Perform specific departmental tasks and/or orient other staff
• Travel between Sacramento Valley Float Pool sites as required

Requirements

• PharmD—Graduate of an accredited pharmacy school
• Registered Pharmacist within 120 days
• Advanced Cardiac Life Support if required by the entity/department
• Pediatric Life Support if required by the entity/department
• 1 year of recent relevant experience
• Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations, and laws and regulations
• Knowledge of different pharmacy practice settings, including narcotic delivery and procedures
• Knowledge of pharmaceutical and medical terms, abbreviations, and symbols used in prescribing, dispensing, and medication record keeping
• Basic working knowledge of legal requirements and accreditation standards including NABP, TJC, Title XXII, DHS, DEA, FDA, and USP
• Written and verbal communication skills to explain sensitive information clearly and professionally to diverse audiences
• Time management and organizational skills, including prioritization and meeting deadlines
• Knowledge of Microsoft Office Suite, pharmaceutical technology, EHR, and EPIC
• Ability to work independently and as part of a team
• Ability to identify, evaluate, and resolve standard problems using established options
• Ability to ensure patient PHI privacy
• Employees may be required to handle hazardous drugs in accordance with USP 800 guidelines
