Qualifications

• Graduate of an accredited School of Pharmacy.
• Active California Pharmacist License.
• Current EPIC experience (required).
• Must successfully complete IV training.
• Experience with inpatient hospital pharmacy.
• Knowledge of pediatric and neonatal medication management.
• Strong clinical decision-making and communication skills.

Responsibilities

• Verify, process, and dispense medication orders accurately and efficiently.
• Monitor medication therapies to optimize patient outcomes.
• Review medication orders for safety, allergies, interactions, dosing, and appropriateness.
• Provide drug information and clinical recommendations to healthcare providers.
• Identify and resolve medication safety concerns.
• Participate in medication management initiatives and clinical pharmacy activities.
• Support quality improvement and regulatory compliance.
• Assist with medication utilization reviews and formulary management.
• Participate in IV compounding services.
• Supervise and support pharmacy technicians.
