Job summary

The Pennant Group, Inc. is seeking a registered nurse to travel among Sweetwater, Uinta, South Lincoln, and Sublette Counties to admit new patients to home health and hospice services.

The ideal candidate is an admissions-focused RN with at least two years of nursing experience, including public health or hospice care, and holds an active RN license with CPR certification. A valid driver’s license and reliable vehicle are required.

Qualifications

• Graduate of National League for Nursing accredited school of nursing.
• Minimum of two years of nursing experience, at least one in public health, home care, or hospice nursing.
• Registered nurse with current licensure to practice professional nursing in the state.
• Maintains a current CPR certification.
• Must have and maintain a valid driver’s license and access to transportation.
• Self-directed and able to work with minimal supervision.

Responsibilities

• Perform initial and ongoing patient assessments and plan of care.
• Provide professional nursing care and administer prescribed treatments.
• Coordinate patient care as case manager for assigned caseload.
• Educate patients and families on self-care and disease management.
• Participate in on-call duties and ensure equipment availability.
