The Pennant Group is seeking an Admissions Registered Nurse for home health and hospice services across multiple locations in Wyoming. The RN will perform new patient admissions, conduct comprehensive assessments, and develop care plans in collaboration with patients and families.

The ideal candidate holds an NLN-accredited nursing program credential, is licensed as an RN, and has strong assessment, communication, and care-planning skills. Travel is required; CPR certification is needed.

Formación

• Graduate of National League for Nursing accredited school of nursing.
• Minimum of two years of experience, at least one in public health, home care, or hospice.
• Management experience not required.
• Registered nurse with current licensure to practice professional nursing in the state.
• Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist.
• Shows ability to organize and prioritize workload independently.
• Prolonged walking or standing; able to lift, position, and transfer patients; CPR certification.
• Must be able to drive and have access to transportation for agency travel; valid driver's license.

Responsabilidades

• Completes initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Develops a care plan and initiates/updates plan of care with patient/family involvement.
• Coordinates care with physicians, team members and community resources.
• Administers medications and treatments as prescribed and monitors patient status.
• Teaches patient and family self-care techniques and disease management.
• Ensures safe environment and continuity of care across settings.
• Participates in on-call duties and case assignments.
• Engages in quality improvement activities.
