Job summary

The Pennant Group, Inc. is seeking an experienced Registered Nurse for Home Health and Hospice in Sublette County, Wyoming.

The RN will assess patients, develop plans of care, and coordinate with an interdisciplinary team to deliver compassionate in-home care. Requirements include an NLN-accredited nursing program graduate, active RN license in Wyoming, CPR certification, and at least two years of public health, home health, or hospice nursing experience.

Qualifications

• Graduate of National League for Nursing accredited school of nursing.
• Minimum of two years of experience in public health, home care, or hospice nursing preferred.
• Registered nurse with current Wyoming licensure.
• CPR certification required.
• Must have valid driver's license and access to reliable transportation.

Responsibilities

• Assess patient and family to determine hospice needs; complete assessments and history.
• Develop and initiate plan of care; revise as needed.
• Coordinate with physician and interdisciplinary team to ensure continuity of care.
• Administer medications and treatments as prescribed; provide patient education.
