Job summary

Optimal Care is seeking a Home Health Occupational Therapist to help patients regain independence in their homes. You will perform functional assessments, design customized treatment plans, and teach adaptive strategies to support safe daily living.

You will collaborate with patients, families, and an interdisciplinary team, travel daily within the service area, and provide education and hands-on training to progress toward greater confidence and independence.

Qualifications

• Graduate of an ACOTE accredited occupational therapy program.
• Current state license as an Occupational Therapist.
• Minimum 1 year of experience as a licensed Occupational Therapist.
• Reliable transportation and valid driver's license.
• Willingness to travel daily for home visits within service area.

Responsibilities

• Assess functional abilities and design personalized treatment plans.
• Develop adaptive strategies and home modifications to support independence.
• Provide education, hands-on training, and encouragement to patients and families.
• Coordinate with an interdisciplinary care team to progress toward independence.
• Document progress and adjust plans to meet patient goals.
