Job summary

VitalCaring is seeking an Occupational Therapist (OT) for Home Health to join our patient-centered care team. This role covers field-based visits, focused on helping patients regain independence and function in daily activities.

You will evaluate, plan, and implement individualized therapy programs, educate patients and families, and collaborate with the care team to ensure smooth transitions and safe home environments.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification and state licensure required.
• Minimum of 1 year clinical experience preferred, home health a plus.

Responsibilities

• Evaluate patients’ functional status and safety at home.
• Develop and implement individualized OT plans with measurable goals.
• Educate patients and families on treatment plans and safety.
• Collaborate with care team to coordinate services and transitions.
• Supervise COTAs as required by state regulations.
