The Springs Living, LLC is seeking a Physical Therapist to deliver high-quality in-house therapy services to adults, with a focus on aging in place. You will assess and treat residents, coordinate care with families and staff, and maintain safety standards.

Travel to communities in Happy Valley and Clackamas areas is required. The role emphasizes patient-centered care, collaboration with the care team, and adherence to professional standards and regulations.

Competenze

• Graduate of an accredited physical therapy program (PT)
• Current state licensure
• Excellent verbal, written and interpersonal communication skills
• Results-oriented with strong critical thinking and problem-solving skills

Mansioni

• Screening new residents to determine intervention needs
• Evaluate residents to collect data for planning and implementation
• Communicate evaluation findings to residents, family and team
• Document treatment times in compliance with laws and regulations
• Coordinate care with insurance reimbursement and billing
• Provide therapy per standards while adapting for individual needs
• Monitor response to intervention and modify as needed
• Educate residents, families and staff on adaptive equipment and devices
• Maintain safety and infection control standards
• Collaborate with community staff and maintain clear communication
