Job summary

The Springs Living, LLC in Beaverton, OR is seeking a Physical Therapist to deliver high-quality in-house therapy services to adults with various diagnoses, focusing on aging in place and mobility, independence, and overall well-being.

You will conduct screenings, evaluate residents, document in accordance with federal and state regulations, coordinate care with families and staff, and educate residents and families on adaptive devices to support daily living.

Qualifications

• Excellent verbal, written and interpersonal communication required.
• Strong critical thinking and problem-solving abilities.
• Ability to coordinate care with residents, families and staff.

Responsibilities

• Conduct screenings of new residents to determine need for intervention.
• Evaluate residents to collect data for treatment planning and implementation.
• Record treatment times and documentation in compliance with laws and regulations.
• Communicate evaluation findings to residents, families, and team members.
• Provide treatment per established standards while adapting to individual needs.
• Monitor resident response and adjust treatment as ordered by physicians.
