Majestic Care of Terre Haute is seeking a licensed Physical Therapist (PRN) to evaluate and implement individualized therapy plans for residents in a skilled nursing facility. The role emphasizes mobility, strength, balance, and functional independence within a collaborative care team.

The PT will educate residents and families, document progress, and ensure compliance with HIPAA, OSHA, and PDPM guidelines while supporting geriatric rehabilitation goals and resident safety.

Formación

• Current state licensure as a Physical Therapist.

Responsabilidades

• Evaluate and plan individualized physical therapy treatments for SNF residents.
• Provide therapeutic exercises, gait and balance training, mobility enhancement, and manual therapy.
• Educate residents and families on home exercises and safety strategies.
• Collaborate with nursing, OT, and SLP to ensure resident-centered care.
• Maintain documentation following HIPAA and PDPM requirements.
