Job summary

Elara Caring is seeking a Speech-Language Pathologist to provide high-quality home-based care across our branch network. You will assess, treat, and document therapy plans for patients in their homes, collaborating with physicians and the care team.

Qualifications

• Licensed SLP in current state
• Minimum 1 year clinical SLP experience
• Willingness to travel within branch/office coverage area
• Valid driver's license and vehicle required
• Ability to lift/push/pull up to 50 pounds

Responsibilities

• Perform direct Speech-Language pathology services per standards
• Admit and evaluate patients and develop a Plan of Care
• Re-evaluate patients during visits and follow PPS/OASIS guidelines
• Communicate findings and changes to supervisor and team
• Document therapy notes and update Plan of Care and orders
