Job summary

Elara Caring is seeking a Speech-Language Pathologist to deliver in-home therapy, evaluate patients, develop and oversee Plans of Care, and coordinate with physicians and care teams. The role emphasizes compassionate care, adherence to OASIS/PPS guidelines, and a commitment to high-quality home-based services.

The position requires a Master’s degree in Language Pathology, an unrestricted SLP license, and travel up to 50% within the service area. A valid driver’s license and vehicle are necessary.

Qualifications

• Master’s degree in Language Pathology from an accredited program.
• Current, unrestricted SLP license in the practicing state.
• Minimum one year of experience as an SLP in a clinical care setting.
• Willingness to travel within branch/office coverage area (up to 50%).
• Capable of lifting up to 50 pounds and assisting with patient transfers.
• Valid driver’s license and dependable vehicle as required for home visits.

Responsibilities

• Promotes Elara Caring’s mission and policies to ensure quality of care.
• Evaluates patients to determine rehabilitation needs and develops a Plan of Care.
• Admits patients and assesses speech-language therapy needs per regulatory guidelines.
• Conducts initial and follow-up therapy evaluations and adjusts the Plan of Care accordingly.
• Performs direct Speech-Language pathology services in home settings per accepted standards.
