Job summary

Apex Home Health, Hospice & Home Care seeks a Speech Language Pathologist (SLP) to evaluate, diagnose, and treat communication and swallowing disorders across diverse patient populations.

You will develop personalized treatment plans, conduct evaluations, document progress, and educate patients, families, and caregivers. Collaborate with interdisciplinary teams to deliver safe, effective care in a home-health setting. Valid license and CCC-SLP preferred.

Qualifications

• Valid state licensure and current BLS certification required.
• Preferred: Master’s degree in Speech-Language Pathology (CCC-SLP).
• Strong communication and clinical skills, with PC proficiency.

Job description

Role: The Speech Language Pathologist (SLP) evaluates, diagnoses, and treats communication and swallowing disorders across diverse patient populations.

Responsibilities: Develops personalized treatment plans, utilizes assessments and clinical observations, monitors patient progress, and maintains detailed documentation. Educates patients, families, and caregivers on communication strategies and safe swallowing practices. Collaborates with interdisciplinary teams to coordinate comprehensive care, adheres to safety and infection control protocols, and engages in professional development. Performs additional duties within scope of practice as needed.

Qualifications & Skills: Valid state licensure and current BLS certification are required. Preferred qualifications include a Master’s or higher degree in Speech-Language Pathology, CCC-SLP, and relevant experience—even new graduates are encouraged to apply. Essential skills include critical thinking, strong interpersonal and communication abilities, proficiency in PC skills, and physical, motor, sensory, and mental capabilities to perform clinical tasks effectively.
