Responsibilities

• Conduct comprehensive speech-language pathology assessments for residents, addressing communication and swallowing needs
• Develop and implement individualized treatment plans; provide direct therapy and educate care staff and families
• Document evaluation results, treatment plans, progress notes, and discharge planning in the medical record with accuracy and compliance
• Collaborate with the interdisciplinary team to optimize patient outcomes; communicate recommendations to nurses, physicians, and other staff
• Perform bedside swallowing assessments; recommend safe diet textures, compensatory strategies, and therapy as indicated
• Support discharge planning and coordinate referrals to outpatient or community services when appropriate
