Responsibilities

• Conduct comprehensive speech-language evaluations and develop individualized treatment plans for residents with communication, cognitive-communication, voice, and swallowing needs
• Deliver direct therapy sessions, monitor progress, and modify treatment plans to maximize functional outcomes
• Manage dysphagia care, including swallow assessments and safety strategies for diet modification when indicated
• Document evaluation findings, therapy interventions, progress notes, and discharge plans in the medical record in a timely, compliant manner
• Collaborate closely with an interdisciplinary team (nursing, rehabilitation services, social work) to coordinate care and support discharge planning
• Provide patient and family education on communication strategies, compensatory techniques, and caregiver guidance to support continued progress

Requirements

• Current, active license to practice as a Speech-Language Pathologist in the United States (licensure specifics to be verified)
• Ability to work in a skilled nursing facility environment with adult patients
