Job summary

American Traveler, LLC. seeks a Pediatric RN Care Coordinator for our Pediatric Complex Care Program. The role blends in-person and remote work, coordinating care for infants to teens and liaising with families.

You will review charts, educate families, triage calls, and collaborate with DME providers, nurses, and other vendors, while maintaining families' ongoing communication.

Qualifications

• Active RN license (MD or Compact) required.
• BLS certification required.
• Strong current pediatric nursing experience required.
• EPIC and case management experience preferred.
• Experience in a comparable large facility setting (500+ bed count) required for consideration.

Responsibilities

• Hybrid role with up to 3 in-person days and 2 remote days weekly.
• Care coordination for pediatric patients and guidance for families.
• Five 8-hour day shifts per week; no weekends, nights, call or holidays.
• Education, after-visit summaries review, and EPIC In Basket triage.
• Coordinate with DME providers, nursing agencies, and other vendors.
• Write family communications and outreach letters; review patient charts.

• Responsibilities include patient education, reviewing after-visit summaries, managing EPIC In Basket messages, triaging calls, and reviewing charts
• Does not include medication reconciliation or refill management
• Coordinates with DME providers, nursing agencies, and other vendors to address barriers to patient care
• Writes family communications and outreach letters as part of care coordination
• Reviews patient charts and manages ongoing communication with families
• Local candidates accepted at the same pay rate
