Job summary

American Traveler, LLC. is seeking a Pediatric Case Management RN with in-patient case management experience for a day-shift role at a major Baltimore hospital. The ideal candidate holds an active MD or Compact RN license, a minimum of 2 years of RN experience, and BLS certification.

Inpatient case management experience and post-acute referral knowledge are required. Responsibilities include floating to NICU, PICU/PCICU, Onc, and other pediatric units, coordinating discharge planning, and

• Minimum 2 years of RN experience required.
• BLS certification required.
• Current in-patient case management experience required.
• Experience with post-acute facility referral platforms such as NaviHealth or AllScripts required.
• Pediatric experience not required, but case management and discharge planning experience is required.
• Bedside experience preferred, ideally 5+ years.
• ICU experience preferred but not required.
• Epic EMR experience preferred.
• Case management certification preferred.
• Experience at a comparable facility size (500+ bed count) required.

Responsibilities

• Pediatric Case Management position at a main hospital facility.
• Will float to all pediatric units including NICU, PICU/PCICU, Onc, and 9N/S and 10N/S units (no float to psych).
• Day shift schedule, 8:30 AM to 5:00 PM, Monday through Friday.
• 40 hours per week with no weekends or holiday coverage required.
• No on-call responsibilities.
• Facilitate discharge planning and coordinate post-acute care referrals for pediatric patients.
• Collaborate with multidisciplinary care teams across various pediatric units.
• Local candidates accepted at the same pay rate.
