Qualifications

• Minimum 1 year of nursing experience required.
• Active, unencumbered Oregon RN license required.
• BSN and/or hospice/home health case management experience preferred.

Responsibilities

• Provide intermittent skilled nursing visits in home-health/hospice settings.
• Coordinate care with a multidisciplinary team across multiple sites.
• Manage pain and symptom control for hospice patients and provide on-call support.
