American Traveler is seeking a Registered Nurse for a Hospice/Palliative Care home care position in New Jersey. Active NJ RN license and two years of home hospice experience are required.

The role involves a hospital-affiliated home care program, 5-day weeks from 8am to 4pm, every other weekend, and comp days. EPIC charting, patient assessments, and interdisciplinary team coordination are essential components of daily duties.

Responsibilities

• Provide home hospice and palliative care in a hospital-affiliated home care program.
• Shift schedule is 5 days/week, 8ap with every other weekend and comp days.
• Floats to counties as needed based on patient demand; may work with a nearby sister hospital.
• No on-call duties.
• Use EPIC for charting and documentation.
• Assess patients to determine hospice suitability and develop care plans with patients and families.
• Complete admissions, discharges, and re-visits within required timeframes.
• Educate patients and families using appropriate teaching tools.
• Manage caseload and coordinate with IDT; attend biweekly IDT meetings.
• Supervise HHAs per standards and ensure proper utilization of services.
• Maintain infection control and reconcile medications.
• Participate in monthly staff meetings and PI activities.
• Provide orientation and mentorship to new staff; cross-train as needed.
• Locals accepted.

• Supervises home health aides (HHAs) according to standards and ensures appropriate utilization of services
• Maintains a safe environment by following infection control protocols and reconciling medications regularly
• Participates in monthly staff meetings and performance improvement (PI) activities
• Provides orientation and mentorship to new employees and cross-trains to learn new skills
