American Traveler is seeking a Clinical Care RN for a PACE clinic and home health setting in Michigan. The role requires an active MI RN license and geriatric care experience.

Schedule is Monday–Friday, 8:00am–4:30pm with on-call/holiday rotation and potential for extended hours. Responsibilities include direct nursing care, wound care, lab draws and assessments during home visits, collaboration with PCPs, and independent management of complex clinical situations.

Qualifications

• Active MI RN license required.
• Minimum 1 year experience with frail, elderly, or long‑term care population.
• Home health experience preferred.
• Wound care experience preferred.
• Current CPR certification by AHA required or willingness to obtain.
• Must have reliable transportation and MI driver's license with car insurance.
• Vaccination against influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B may be required.

Responsibilities

• Provide direct nursing care in clinic and home visits including wound care and nurse assessments.
• Coordinate monitoring of acute and chronic conditions with Primary Care Providers.
• Triages emergent situations and administers prescribed treatments and medications.
• Work independently applying advanced nursing knowledge in complex clinical scenarios.
• Maintain presence within the center and communicate with staff, participants, and caregivers.
• On-call and holiday rotation; may work beyond normal hours including weekends and holidays.
