Job summary

American Traveler is seeking a Hospice RN to perform home health visits across the Petoskey, MI region. The role requires 2+ years of hospice experience and proficiency with Home Care Home Base.

You will cover the West side of the area, with day shifts from 8:00 AM to 5:00 PM, and a weekend rotation. Travel is required, with first-time travelers welcome and a local radius consideration.

Qualifications

• Active MI RN license required at time of submission
• Compact license not accepted
• BLS certification (AHA)
• Minimum 2 years of hospice experience required
• Home Care Home Base experience required
• Oasis experience preferred
• Travel nursing experience preferred

Responsibilities

• Provide home health nursing care to hospice patients in their homes
• Covers the West side coverage area including Petoskey, East Jordan, Boyne City, Harbor Springs, and Mancelona, with potential coverage up to Mackinaw City along I-75 West
• Day shift schedule, 8:00 AM-5:00 PM
• Weekend rotation required
• No on-call requirements
• Uses Home Care Home Base EMR system
• Point of Care documentation practice
• Provides skilled nursing care and health counseling to patients in their homes to support treatment, rehabilitation, and symptom management
• Conducts initial visits to assess patient physical and emotional status through observation, interview, and record review
• Develops nursing care plans in accordance with physician treatment plans and provides patient teaching
• Works under the supervision of the Clinical Services Supervisor
• Matching scrubs required for dress code
• Candidates residing within a 35-mile radius of the facility are considered local travelers
• First-time travelers are welcome to apply
