HealthForce Healthcare is seeking a Travel Registered Nurse for a 13-week assignment in Glendale, CA. The role emphasizes care coordination and case management within a hospital setting, with a 40-hour week and day shift (Sun-Thu, 8:00 AM–4:30 PM). Start date 11/02/2026, potential extension, comprehensive credentialing support, and dedicated recruiter assistance.

Ideal candidates hold CA RN licensure, BLS, 2+ years of recent nursing experience; BSN preferred and case management experience a plus.

Qualifications

• Current Registered Nurse license in California.
• Basic Life Support (BLS) certification required.
• Minimum 2 years of recent nursing experience.
• BSN preferred.
• Case management experience preferred; case management certification preferred.

Responsibilities

• Lead coordination of patient care with the multidisciplinary team; monitor care appropriateness and timeliness.
• Act as a liaison among patients, families, nurses, physicians and care team to optimize outcomes.
• Guide care management from pre-admission through post-discharge, including discharge planning and resource discussions.
• Manage care for complex patients; coordinate transfers with case management.
• Provide oversight for regulatory and accreditation data collection and transfer of information.
