Job summary

HealthForce Healthcare in Glendale, California is seeking a Registered Nurse – Case Management for a travel, temporary assignment. Start date 10/12/2026 with an expected 13-week contract. Weekly pay ranges from $2,766 to $2,900. Shift: Days, 8 hours per day (40 hours per week).

The role involves coordinating a continuum of care, liaising with the care team, and supporting patient outcomes in a hospital setting. Travel and housing guidance are provided as part of recruiter support.

Qualifications

• Active registered nurse license in California (CA)
• Current Basic Life Support (BLS) certification
• Prior case management or care coordination experience preferred
• Excellent communication and collaboration skills with multidisciplinary teams
• Strong clinical assessment and care planning abilities

Responsibilities

• Provide patient care by supporting the development, implementation, revision and reporting of the case management program
• Act as a liaison between patient, family, nurse, physicians, and the care team to optimize outcomes
• Serve as a consultant to the healthcare team on patient items
• Coordinate a continuum of care from pre-admission through post-discharge
• Ensure patients and families move efficiently through hospitalization
