• Manage care for complex patients; coordinate transfers with case management.
• Provide oversight for regulatory and accreditation data collection and transfer of information.

Responsibilities

• Lead coordination of patient care with the multidisciplinary team; monitor the appropriateness and timeliness of care; ensure the interdisciplinary care plan aligns with the patient's clinical course and discharge needs
• Act as a liaison among patients, families, nurses, physicians and the care team to optimize outcomes and ensure continuity of care
• Guide the care management process from pre-admission through post-discharge, including discharge planning and conversations about resource utilization with the medical team
• Manage care for complex or high-utilization patients, collaborating with case management for ongoing planning and coordinating transfers when needed
• Provide oversight for regulatory and accreditation data collection and ensure accurate transfer of information to support ongoing treatment and continuity of care

Requirements

• Current Registered Nurse license in the state of practice (California required)
• Basic Life Support (BLS) certification required
• Minimum 2 years of recent nursing experience
• BSN preferred
• Case management experience preferred; Case management certification preferred
• Ability to operate effectively in a hospital setting and participate in a travel assignment
