Healthforce seeks a Registered Nurse, Geriatric for a Glendale, CA hospital travel assignment starting 10/12/2026. This 13-week assignment offers day shifts, 8:00 AM – 4:30 PM, 36 guaranteed hours per week, and weekly pay of $2,804–$2,937.

The role emphasizes coordinating comprehensive geriatric care from pre-admission through post-discharge, with lead- and team-based planning, collaboration with physicians, social workers and care teams, and smooth transitions of care.

Qualifications

• RN license in California is required.
• BLS certification required.
• BSN is preferred.
• Minimum two years of recent nursing experience; care management experience preferred.

Responsibilities

• Coordinate and manage comprehensive geriatric care from pre-admission through post-discharge.
• Lead interdisciplinary care planning to align with patient’s clinical course and needs.
• Collaborate with physicians, nurses, social workers and other care team members to optimize outcomes.
• Monitor diagnostics and treatments; communicate plan adjustments and discharge planning.
• Focus on complex cases and facilitate smooth transitions of care; support transfers to tertiary centers when needed.
