Job summary

Healthforce invites applications for a Registered Nurse – Case Management in Glendale, CA, a travel temporary hospital assignment starting 2026-10-12 for 13 weeks. The position offers weekly pay between $2,766 and $2,900, with days shift (8 hours, 40 hours per week).

You will coordinate care with physicians, nurses and the multidisciplinary team, guiding patients from pre-admission to post-discharge and ensuring efficient progression and positive outcomes in a hospital setting.

Qualifications

• Active registered nurse license in California (CA)
• Current Basic Life Support (BLS) certification
• Prior case management or care coordination experience preferred

Responsibilities

• Provide excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program.
• Act as a liaison between patient, family, nurse, physicians, multidisciplinary team and patient’s healthcare benefactor to optimize outcomes.
• Serve as a consultant to the healthcare team on specific patient items.
• Coordinate a continuum of care for a defined population from pre-admission through post-discharge.
• Ensure patients and families proceed efficiently through the course of hospitalization.
