Job summary

HealthForce Healthcare is seeking a Travel Registered Nurse (RN) – Long-Term Care (LTC) for Glover, VT. The ideal candidate delivers resident-centered care in skilled nursing settings and collaborates with physicians, therapists, and families.

This full-time travel assignment features a 40-hour week with a mix of 12- and 8-hour shifts, with every-other weekend. BLS and an RN license (VT or compact) are required, plus LTC/SNF experience and strong documentation in EHR.

Qualifications

• ADN or BSN from an accredited program
• RN license valid in VT or a compact/NLC
• BLS certification required or active
• Geriatric or LTC/SNF experience preferred
• Ability to travel and adapt to new assignments

Responsibilities

• Perform comprehensive physical and health assessments, closely monitoring residents for changes in condition
• Administer medications and manage IV therapy according to prescribed orders and facility protocols
• Provide skilled wound care, skin integrity management, and pressure ulcer prevention strategies
• Identify and elevate acute clinical changes immediately, updating care plans in collaboration with the interdisciplinary team
• Coordinate care plans alongside physicians, physical/occupational therapists, and social workers
• Maintain timely, accurate, and thorough documentation of all care, treatments, and resident responses in the Electronic Health Record (EHR) system
• Provide compassionate disease management education and safety guidance to residents and their families
