Qualifications

• Education: ADN or BSN from an accredited program.
• Licensure: Active RN license in Vermont or compact/NLC.
• Certifications: BLS required; CGN or gerontology credential preferred.

Responsibilities

• Perform comprehensive physical and health assessments, monitoring residents for changes.
• Administer medications and manage IV therapy per orders.
• Provide skilled wound care and skin integrity management.
• Identify acute changes and update care plans with the interdisciplinary team.
• Coordinate care plans with physicians, therapists, and social workers.
• Document care and resident responses accurately in the EHR.
• Educate residents and families on disease management and safety.
