Job summary

HealthForce Healthcare seeks an experienced Travel Registered Nurse (RN) for a long-term care setting in Dubuque, IA. You will deliver high-quality nursing care focusing on chronic disease management for frail older adults in a dynamic facility.

The role requires ADN/BSN, Iowa or Compact RN licensure, BLS, and LTC/SNF experience with EHR/EMR proficiency. This is a full-time, contract travel position with day shifts.

Qualifications

• Education: ADN or BSN from an accredited program
• Licensure: Active, unencumbered RN license valid in Iowa or active Compact/NLC RN license
• BLS certification required; CGN or gerontology credential preferred
• Experience in LTC/SNF; chronic disease management; proficiency with EHR/EMR
• Strong communication and interpersonal skills

Responsibilities

• Perform comprehensive physical assessments and monitor residents for acute changes
• Administer medications and manage IV therapy per orders and facility protocols
• Provide routine wound care and skin integrity management including pressure ulcer prevention
• Document care, treatments, and resident responses in the EHR/EMR
• Collaborate with physicians, therapists, and social workers to tailor care plans
• Educate residents and families on disease management and medication administration
• Facilitate transitions of care within facility or to higher-level care
