Job summary

Transcend-Hospice seeks a registered nurse to administer skilled nursing care in patients' homes and coordinate with the interdisciplinary team, patient, family, and referring agency. The role emphasizes end-of-life palliation and comprehensive assessment to guide personalized care plans.

The RN must hold an active Texas license, have reliable transportation, and be able to travel locally. Hospice or home health experience is preferred.

Qualifications

• Nursing skills per standards of practice.
• Excellent interpersonal and communication abilities.
• Proof of Hepatitis consent/declination.
• Palliation and end-of-life care skills preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients.
• Develop and update the plan of care with the interdisciplinary team.
• Educate patients and families about disease process and end-of-life care.
• Provide training to staff as needed.
