Job summary

CVS Health in Dallas, TX seeks a Registered Nurse Case Manager to provide high-quality case management via telephonic and in-person visits, helping members navigate healthcare needs and improve outcomes.

You will assess, plan, coordinate, and monitor care; educate members; document accurately; and mentor new staff. Requirements: Texas RN license, an Associate degree, 3+ years in clinical nursing, 1+ year in case management; travel up to 75%.

Qualifications

• Associates Degree in Nursing required.
• Bachelor’s in Science & Nursing preferred.
• Licensed Registered Nurse in Texas required.
• 3+ years of clinical nursing experience.
• 1+ year of case management experience.

Responsibilities

• Conduct comprehensive telephonic and in-person assessments.
• Develop and implement individualized care plans.
• Coordinate and monitor ongoing care plan activities.
• Collaborate in multidisciplinary care reviews to optimize member health results.
• Educate and empower members to participate in healthcare decision-making with their providers.
• Use documentation standards to ensure accurate, timely records.
