Job summary

CVS Health in Northern Virginia is seeking a compassionate Registered Nurse Case Manager to join our team. This field-based role blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care. Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST.

The role requires an active Virginia RN license, 3+ years of clinical nursing experience, and willingness to travel up to 75%. Bachelor’s degree in nursing is preferred. Anticipated weekly hours: 40.

Qualifications

• Active, unrestricted RN license in Virginia.
• Residence in Northern Virginia.
• Willingness to travel up to 75% with mileage reimbursed.
• 3+ years of clinical nursing experience (medical-surgical, acute care, or behavioral health).
• Strong communication, organization, and critical-thinking skills.
• Comfort working with multiple systems and documentation platforms.

Responsibilities

• Conduct comprehensive assessments (telephonic and face-to-face) to evaluate medical, behavioral, and social needs.
• Develop and implement personalized care plans promoting short- and long-term wellness.
• Coordinate care across providers, programs, and services for a seamless member experience.
• Use clinical expertise and data insights to identify risks and gaps in care.
• Support members with complex conditions using a holistic, whole-person approach.
• Facilitate referrals to clinical and community resources.
• Participate in case conferences and collaborate with multidisciplinary teams.
