Job summary

CVS Health in Alexandria, VA seeks a Registered Nurse Case Manager to deliver coordinated, patient-centered care through in-person visits and telephonic outreach. You will assess medical, behavioral, and social needs and develop individualized care plans.

The role requires 3+ years of clinical nursing, an active Virginia RN license, and reliability with travel up to 75%. Schedule is Monday–Friday, 8:00 AM–5:00 PM EST with comprehensive benefits.

Qualifications

• Associate Degree in Nursing is required.
• Virginia RN license in good standing.
• 3+ years of clinical nursing experience requested.

Responsibilities

• Conduct comprehensive assessments (telephonic and face-to-face) to evaluate members’ medical, behavioral, and social needs.
• Develop and implement personalized care plans that promote both short- and long-term wellness.
• Coordinate care across providers, programs, and services to ensure a seamless experience for members.
• Use clinical expertise and data insights to identify risks, gaps in care, and opportunities for intervention.
• Support members with complex or multiple conditions by applying a holistic, whole-person approach.
• Facilitate referrals to appropriate clinical and community resources.
• Participate in case conferences and collaborate with multidisciplinary teams to optimize care strategies.
• Review claims history and benefit eligibility to inform care planning.
• Engage members using strong interviewing skills to build trust and encourage active participation in their care.
• Ensure all case management activities comply with regulatory and organizational guidelines.
