Job summary

American Traveler is seeking an ED RN Care Coordinator/Case Manager with recent CA RNCC experience in a Dignity or CommonSpirit facility and an active CA RN license. The role focuses on rapid assessment, utilization review, and discharge planning within the Emergency Department.

You will manage 15-20 patient consults daily across payer types, using InterQual criteria and Cerner EMR, with a daytime shift and weekend coverage as required. Training and support are provided in a hospital setting.

Qualifications

• Active CA RN license required.
• BSN required.
• BLS (AHA) certification required.
• Minimum 2 years of RN experience required.
• Recent RN Care Coordinator (RNCC) experience in a Dignity or CommonSpirit facility is mandatory.
• Prior ED Care Coordination experience required.
• Experience with Cerner EMR required.

Responsibilities

• Acute care, Level 2 Trauma hospital setting within the Care Coordination/Case Management department, specifically assigned to the Emergency Department.
• Handles 15-20 patient consults per day across all payer types, including uninsured and homeless populations.
• Initial assessments, utilization review (UR), and discharge planning using InterQual criteria.
• Covers all areas of the hospital with no outpatient or home care responsibilities.
• Uses Cerner EMR for charting and InterQual for UR.
• Shift schedule is 1200-2030 with hours generally between 8am-5pm.
• Requires working every other weekend, with at least one full weekend per month.
• On-site Hospitalist/Intensivist and 24/7 Respiratory Therapy support available.
• Role involves UR and discharge planning duties.
• Training on InterQual will be provided.

• Minimum 2 years of RN experience required
• Recent experience as an RN Care Coordinator (RNCC) within a Dignity or CommonSpirit facility is mandatory for consideration
• Prior ED Care Coordination experience required
• Familiarity with InterQual criteria required
• Experience with Cerner EMR required
• Two professional references required: one supervisor reference from within the last 12 months worked, and one peer or supervisor reference from within the last 3 years worked, both including employment dates and rehire eligibility
• Copy of driver's license required to validate legal name matches government-issued ID

• Business casual attire required
• 5 days of care coordination new employee orientation provided; unit orientation length varies case by case
• Candidates must not have been directly employed (full-time, part-time, or PRN) by any CommonSpirit-affiliated facility, including CHI or Dignity, within the past year; PRN employment through outside staffing firms is acceptable
• Hospital may cancel up to 4 shifts per 13-week assignment
