Molina Healthcare seeks a field-based RN Care Manager to support LTSS member care across the continuum. You will collaborate with multi-disciplinary teams to ensure quality, cost-effective care and facilitate waiver services.

Expect travel within Black Hawk County and the ability to engage diverse populations. Required are an active RN license, 2+ years in health care with care management, strong communication, and proficiency with MS Office.

Formación

• RN license active and unrestricted in state of practice.
• At least 2 years in health care with care management or LTSS exposure.
• Experience with disability/ chronic conditions or substance use disorders is a plus.
• Proficient in Microsoft Office and standard health IT tools.

Responsabilidades

• Conduct comprehensive member assessments within timelines, including home visits.
• Coordinate waiver enrollment/disenrollment and care planning with the team.
• Monitor care plans, document interventions, and adjust as needed.
• Promote integrated LTSS with behavioral health and community resources.
• Perform medication reconciliation and ICT collaboration as required.
• Travel 25-40% locally to support members in Black Hawk County.
