Qualifications

• Bachelor's degree from an accredited education program in a human services field; OR a bachelor's degree in a non-human services field, with two years of experience in a human services field.
• Experience working with people with developmental disabilities.

Responsibilities

• Provide Case Management services to individuals with intellectual and/or developmental disabilities.
• Establish positive working relationships and plan for, monitor, and evaluate Person Centered Plans.
• Identify and secure ongoing support services and coordinate resources for PCPs with external stakeholders.
• Ensure services meet state-mandated standards and SCI policies.

Education
