Qualifications

• Active LPN license in the state of employment.
• Current BLS/CPR certification.
• Current TB test/results.
• Current physical stating fit for duty (within the past year).

Responsibilities

• Provide direct nursing care to residents under supervision of an RN.
• Administer medications and treatments per physician orders and facility policies.
• Monitor and document residents' conditions, reporting changes promptly.
• Collaborate with physicians, nurses, therapists and other healthcare professionals to ensure quality patient care.
• Educate residents and families on health conditions, medications, and ongoing care needs.
• Maintain accurate and timely documentation per facility and regulatory standards.
