Qualifications

Active LPN license in the state of employment (or ability to obtain).
Current BLS/CPR certification.
Current TB test/results.
Current physical stating fit for duty (within the past year).

Responsibilities

Provide direct nursing care to residents under the supervision of an RN.
Administer medications and treatments according to physician orders and facility policies.
Monitor and document residents' conditions, reporting any changes promptly.
Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure quality patient care.
Educate residents and families on health conditions, medications, and ongoing care needs.
Maintain accurate and timely documentation in accordance with facility and regulatory standards.
