Job summary

Healthforce Healthcare LLC is recruiting a Physical Therapy Assistant for a travel assignment in Hampton, NH. Start date is 11/23/2026 for a 13-week contract, with 35 guaranteed hours per week and a 5x8 shift (08:00–16:30).

The position operates in a LTAC-focused long-term care facility with an assisted living component and supervision by a licensed physical therapist. You will deliver PTA services, monitor and adjust treatment plans, assist with assessments, documentation, and discharge

Qualifications

• Active Physical Therapy Assistant license in New Hampshire or eligibility to obtain licensure per state regulations.
• Associate degree from an accredited PTA program.
• Previous LTAC or skilled nursing experience preferred.
• Strong communication and teamwork; comfortable documenting in electronic health records.
• BLS/CPR as required by employer and facility policy.

Responsibilities

• Deliver PTA services under the supervision of a licensed physical therapist to residents in a LTAC/long-term care setting
• Implement and monitor treatment plans including therapeutic exercises, gait training, mobility training, and activities of daily living tasks
• Assist with patient assessments, document progress, and communicate findings to the supervising PT and care team
• Ensure patient safety during therapy sessions, apply proper body mechanics, and adhere to infection control and safety protocols
• Collaborate with nursing staff and family members to support rehabilitation goals and discharge planning
• Maintain accurate, timely documentation in patient records according to facility policies and regulatory requirements
