Healthforce Healthcare LLC seeks a licensed Physical Therapy Assistant for a 13-week LTAC travel assignment in Salem, New Hampshire. Start date 10/19/2026. 36 guaranteed hours per week, shift 08:00-16:30, in a Skilled Nursing Facility setting. The role is travel-based and managed through Healthforce Healthcare LLC.

You will provide skilled PT services under supervision, document progress, collaborate with the care team, and support mobility and daily living activities in LTAC-level care.

Qualifications

• Active PTA license in NH
• Graduation from an accredited PTA program (associate degree)
• CPR/BLS certification required
• Ability to work in a skilled nursing LTAC environment

Responsibilities

• Provide skilled physical therapy services to LTAC patients under the supervision of a licensed physical therapist.
• Implement and progress treatment plans, monitor patient response, and adjust as directed.
• Document patient assessments, treatment sessions, and progress in the medical record.
• Collaborate with an interdisciplinary team to support mobility, transfers, gait training, and functional goals.
• Support activities of daily living, device use, and safe wheelchair positioning; maintain equipment and perform safety checks.
• Adhere to infection control, safety standards, and facility policies; communicate patient status and treatment updates clearly.
