Qualifications

• Licensed PT or eligible to practice in the state.
• Graduate of an accredited PT program.
• Interest or experience in geriatric rehab and long-term care.

Responsibilities

• Evaluate patients and provide personalized physical therapy to improve mobility and independence.
• Develop and implement patient-centered treatment plans for long-term goals.
• Deliver hands-on therapies to support recovery and functional outcomes in older adults.
• Assist patients to safely perform daily activities and enhance confidence.
• Collaborate with interdisciplinary teams to drive meaningful outcomes.
• Track progress and document clinical notes, updating plans as needed.
