Job summary

Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist (PT) to deliver patient-centered care to adult and geriatric populations. You will promote independence, mobility, safety and quality of life while working in a collaborative clinical environment.

The role emphasizes clinical autonomy and outcomes, with flexible scheduling and a focus on high-quality care for older adults in skilled nursing or long-term care settings.

Qualifications

• Licensed PT in the state of practice
• Graduate of an accredited Physical Therapist Program
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
• Passion for patient-centered care and improving quality of life for older adults

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and functional independence
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals
• Deliver hands-on therapy interventions to support recovery and enhance quality of life in older adults
• Help patients restore functional mobility and safely perform daily activities with greater confidence and independence
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs
