Job summary

Therapy Management Corporation is seeking a licensed Physical Therapist to provide patient-centered care for adult and geriatric populations in a collaborative clinical setting. You will evaluate, treat, and monitor patients to improve mobility, strength, balance, and independence while maintaining high standards of clinical autonomy.

Qualifications

• Licensed PT (state license held or eligible)
• Graduate of an accredited Physical Therapist program
• Interest or experience in skilled nursing or geriatric rehabilitation
• Passion for patient-centered care and improving quality of life for older adults

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and independence
• Develop and implement patient-centered treatment plans for long-term rehab goals
• Deliver hands-on therapy interventions for recovery and endurance in older adults
• Help patients restore functional mobility for daily activities with confidence and independence
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive outcomes and continuity of care
• Track progress, maintain accurate documentation, and adapt plans as needed
