HealthForce Healthcare in Bedford, New Hampshire is seeking a Physical Therapy Assistant for a 13-week LTAC travel assignment. Start date is 11/16/2026 with a 35-hour week and a fixed shift from 08:00 to 16:30.

Responsibilities include providing PT services under supervision, adjusting treatments for LTAC patients, and documenting progress. Strong communication and teamwork are essential, with onboarding and ongoing support provided by the agency.

Qualifications

• Active Physical Therapy Assistant license in New Hampshire or eligibility to obtain licensure.
• LTAC or skilled nursing setting experience preferred.
• Availability to start on 11/16/2026 and commit to a 13-week travel assignment.
• Ability to work the shift 08:00–16:30 with guaranteed 35 hours per week.
• Strong communication, documentation, and collaborative skills for interdisciplinary care.

Responsibilities

• Provide physical therapy services under supervision, following the plan of care developed by the supervising PT.
• Implement and adjust treatments for LTAC patients, including gait training, ROM, strength, and ADLs.
• Monitor patient progress, document responses to treatment, and communicate updates to the PT and interdisciplinary team.
• Maintain accurate documentation in the patient record and ensure compliance with regulatory and facility policies.
• Educate patients and families on exercises, safety, and home exercise programs; promote patient-centered care and empowerment.
• Participate in rounds and care coordination with nursing, therapy, and rehabilitation teams; assist with equipment maintenance and infection control.
