Job summary

Healthforce Healthcare LLC is seeking a traveling Physical Therapist for a skilled nursing facility in New Hampshire for a 13-week assignment starting 10/26/2026, with guaranteed 35 hours weekly and a 5x8 day shift (08:00–16:30).

The role includes initial evaluations, treatment plan development, therapy to improve mobility and independence, regular reassessments, and collaboration with an interdisciplinary team.

Qualifications

• Current, unrestricted license to practice physical therapy in the United States.
• Doctor of Physical Therapy (DPT) degree or equivalent.
• Experience in a skilled nursing facility or long-term care setting preferred.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Conduct initial PT evaluations and develop treatment plans for residents in LTC.
• Deliver therapy to address mobility, strength, balance, gait, and independence goals.
• Regularly reassess and update goals and treatment plans based on progress.
• Document assessments, sessions, and progress notes per regulatory standards.
• Collaborate with interdisciplinary team for care coordination and discharge planning.
• Educate residents and families on exercises, safety, and home programs.
