Healthforce is seeking a Physical Therapist for a 13-week travel assignment in Lodi, California. Start date 08/31/2026. Work 36 hours per week on a 4×10-hour day schedule in a short-term acute care hospital with outpatient caseload.

The role involves evaluating patients, planning treatment programs, collaborating with an interdisciplinary team, and maintaining accurate documentation. CA licensure and a DPT are required; CPR/BLS and outpatient experience preferred; Healthforce provides travel

Formación

• DPT degree required; license to practice in CA.
• CPR/BLS certification from approved vendor required.
• Two years of Physical Therapy experience preferred.
• Ability to work in outpatient caseload and 4×10-hour shift schedule; collaborate within an interdisciplinary team.

Responsabilidades

• Plan and conduct segments of treatment programs to restore and improve physical function.
• Perform initial evaluations, ongoing re-assessments, and establish individualized treatment plans.
• Provide therapy interventions and educational guidance per approved protocols; adjust plans as needed.
• Serve as co-leader in an interdisciplinary team approach for patient groups; coordinate with other disciplines.
• Develop individualized care and discharge plans; communicate plans to patients and care teams.
• Complete comprehensive clinical documentation within deadlines; supervise assigned assistants.
