TEEMA Solutions Group in Lodi, CA is seeking a Travel Physical Therapist for a 13-week assignment in an Acute/Rehab setting. You will evaluate and treat patients, develop care plans, and collaborate with the interdisciplinary team to improve mobility and safety.

This role offers a day shift, 8-hour schedule, with weekly pay and benefits options. California license and BLS/CPR are required, along with recent PT experience.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license required.
• Current BLS/CPR certification as required by the assignment.
• Recent physical therapy experience appropriate to the assigned setting.
• Strong assessment, treatment planning, documentation and patient-care skills.

Responsibilities

• Perform physical therapy evaluations and establish individualized treatment plans and functional goals.
• Provide therapeutic exercise, mobility training, functional activities, and other appropriate interventions.
• Monitor patient response to treatment and modify the plan of care as needed.
• Educate patients, families, and caregivers regarding exercises, safety, mobility, and continued care.
• Complete accurate and timely clinical documentation and collaborate with the interdisciplinary care team.

Job Summary

Evaluate and treat patients requiring physical therapy services in a healthcare rehabilitation setting. Develop individualized plans of care, provide therapeutic interventions, educate patients and caregivers, and collaborate with the interdisciplinary team to improve mobility, function, and safety.

Job Qualifications
