Healthforce is seeking a Physical Therapy Assistant for LTAC support in Santa Clara, CA. This travel, temporary assignment runs 13 weeks starting 12/28/2026 with weekly pay between $1,729 and $1,885 and guaranteed 35 hours per week.

Shift 08:00–16:30, 5x8 days in a Skilled Nursing Facility with LTAC responsibilities. You will provide PTA services under licensed PT oversight, assist with mobility and functional activities, document progress, and coordinate care with the nursing team.

Qualifications

• Active California Physical Therapy Assistant license
• Graduation from an accredited PTA program; Associate Degree in Physical Therapy Assisting
• CPR/BLS certification

Responsibilities

• Provide PTA services under licensed PT oversight, implementing treatment plans for LTAC/SNF patients
• Assist patients with mobility, transfers, gait training, balance, and functional activities
• Administer therapeutic exercises, range of motion, strengthening, and functional training as prescribed
• Monitor patient progress, document outcomes, and communicate changes to the supervising PT
• Prepare treatment areas, set up equipment, and maintain safety and infection control standards
• Collaborate with nursing staff and the broader care team to coordinate care and discharge planning

Requirements
