Job summary

Apex Home Health, Hospice & Home Care in Weatherford, TX, seeks an Outpatient Physical Therapist (Travel) to evaluate and treat patients recovering from injuries and managing chronic conditions in an outpatient setting. Qualifications

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program.
• Active state license to practice physical therapy.
• BLS certification may be required.

Role: Outpatient Physical Therapist (Travel)

• Responsibilities: Assess and treat patients recovering from injuries or managing chronic conditions in a outpatient setting; conduct evaluations, develop treatment plans, and assist with patient recovery; collaborate with support staff and utilize available tech resources.
• Work Environment: 5x8-hour shifts in a 5,000 sq ft outpatient facility offering aquatics, sports, and community care services; manage a patient volume of 13-15 daily with 2-3 evaluations per day.
• Qualifications & Skills: Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program; active state license; BLS certification (may be required); experience with Web PT EMR with AI.
• Key Duties: Provide patient-centered therapy, perform evaluations, support recovery, and work efficiently within a team, with assistance from techs as needed.
